How to surrender the LIC Policy Documents for the maturity.

Surrender the policy just before 1 or 2 weeks after considering the maturity date of the policy date. 

a. Fill the Neft intimation Mandate form which is given by LIC 

b. Or fill the red coloured Discharge Farm.

c. Revenue Ticket to be sticky  and.to be signed by the policy holder.

d. Witness box to be dully filled by agent. 

e. Signed Cancelled Cheque to be submitted with mentioned name, account  and IFC code.

If lieu of check the first page of Bank Passbook to be witnessed by the agent.

f. A dully self attested Pan Copy is must if the payment worth stands more than 50000/-

g. A dully self attested  Aadahr ka Photo copy self attested.

h. Original Copy of the Documents to be attached or surrendered.

i. Witness required on  Adhar , Pan copy and Discharge Farm. 

107/20 Document to be submitted original. 75/20   Document to be submitted original.

h.in Exclusion of Jeevan  Anaand ,Policy No 149, Jeevan Surabhi table no 106 / 107 /108 as insurance exists for 100 years then only Photo Copy is required.
